How to Access Low Vision Services

Our Low Vision Services are available through various referral sources:

Medical Referrals:

  • Ophthalmologists: Eye care specialists can refer patients for low vision rehabilitation
  • Optometrists: Primary eye care providers can recommend low vision services
  • Primary Care Physicians: General practitioners can refer patients for evaluation

Self-Referral:

Individuals can also self-refer for low vision services. Simply contact our office to schedule an initial consultation and assessment.

Service Locations:

  • Tampa Location: Serving Hillsborough County residents
  • Winter Haven Location: Serving Polk and Hardee County residents

Note: A recent eye examination (within 6 months) is recommended before beginning low vision services.

Eligibility

Who Qualifies for Low Vision Services?

Our Low Vision Services are available to individuals who:

  • Have a diagnosed visual impairment that cannot be fully corrected with glasses or contact lenses
  • Are residents of Hillsborough, Polk, or Hardee counties
  • Have remaining vision that can be enhanced with aids and techniques
  • Are motivated to learn and use low vision aids
  • Can benefit from vision rehabilitation services
