Service Locations

Two Convenient Locations

We provide on-site comprehensive rehabilitation training in two locations across Florida:

  • Tampa Location: Serving Hillsborough County residents
  • Winter Haven Location: Serving Polk and Hardee County residents

Our training programs are designed to help individuals who have recently lost part or all of their vision to gain the skills needed to perform daily living tasks independently and maintain their employment through the use of assistive technology and devices.

Eligibility Guidelines

Who Qualifies for Our Services?

Our services are available to individuals who meet the following criteria:

Primary Eligibility Requirements:

  • Visual Impairment: Have an uncorrectable visual impairment that significantly affects daily activities
  • Residency: Be a resident of Hillsborough, Polk, or Hardee counties in Florida
  • Impact on Function: Experience interference with employment, education, or daily living tasks due to vision loss
  • Motivation: Demonstrate willingness to participate in rehabilitation services

Age-Specific Requirements:

  • Early Intervention: Children ages 0-5 with visual impairments
  • Children’s Program: Children ages 5-13 with visual impairments (Learn More)
  • Transition for Teens: Teens ages 14-21 preparing for adulthood
  • Adult Services: Adults 18+ seeking employment or independent living skills

Documentation Required:

  • Recent eye examination report (within 6 months)
  • Proof of residency in service area
  • Medical documentation of visual impairment
  • Referral from medical professional (recommended)
